Our Adult Day Support services strive to help individuals build connections in their community, help them practice, learn, and build upon their activities of daily living skills (ADLs), and allow them to be involved in a variety of activities to encourage personal and social growth, as well as independence.
At Eastwood Day Services, our compassionate staff strive to make every moment a teaching moment—where individuals can grow to their highest potential.
Our mission is to help individuals build connections in their community, help them practice, learn, and build upon their activities of daily living skills, or ADLs, and allow them to be involved in a variety of activities to encourage personal and social growth, as well as independence. This unique daytime care is provided at a specialized facility outside of the individual’s home, where their desires and needs are met in a friendly, safe environment. Although limited, transportation is provided in our modified vehicles.
This licensed provider facility includes adaptive equipment to service an array of physical needs. Our specialized restrooms are spacious and equipped with showers, a changing station, and ceiling and electronic hoyers to assist with varying needs.
Our adult day support services use appropriate structured environments with plans to match the desired outcomes of all individuals.
Understanding
It is important for our organization and its staff to understand developmental disabilities and/or physical needs of each individual in attendance. Understanding what the person desires and needs in order to learn and grow is essential in understanding the plan development of each individual. Understanding physical needs is important to building, creating, and maintaining a facility or space for the individual to be able to participate in activities, be social, and to be safe.
Focused Services
It is important that each service we provide focuses on areas such as: fine motor skill and development, cognitive and memory, artistic and sensory, and physical well-being. Each service should focus on whether a disability is intellectual, sensory, metabolic, or degenerative (or a combination of these), and promote activities in these categories for increased participation: recreation, such as board and trivia games, dodge ball, cornhole; exercise, such as walking on a treadmill or outside, arm curls, or other activities adapted for individuals who are non-ambulatory; social, such as coffee breaks, card games, TV, community services/activities; creative, such as building models, playing instruments, or arts and crafts.
Structured Plans
Each individual has a structured plan with our organization. The structured plan includes the individual’s Individual Service Plan (ISP), their structured plan service documentation, and their Goal Achievement Plan (GAP).
The Individual Service Plan is a document created by the individual’s team, which includes their Service and Support Administrator (SSA), guardians, family members, natural supports, and their residential and day program providers and is an important guide that helps staff to understand the individual.
The service documentation focuses on how to support the individual through proper communication techniques, proper ambulatory assistance, eating assistance, maintaining good health, toileting assistance, financial assistance, safety drills, and goals with action steps for achievement.
The Goal Achievement Plan focuses on combining their desired outcomes with areas such as sensory, fine motor, cognitive/memory, problem solving, safety, and ADL skills to promote their continued knowledge and their independence.
The structured program and plans will use a calendar or structured “task boxes” to promote building and following schedules. The monthly activities calendar will include Goal Achievement Plan sessions, exciting group activities, classroom sessions, fun community outings, themed dances, and therapies.
Our new facility offers many amenities in order to support the structured plans of each individual in attendance. Whether its arts and crafts, community involvement, medical services, or educational activities, we have the ability to provide the means to support them.
